RESPONSIBLE FOR:
The discharge coordinator is responsible for supporting the transplant team by ensuring a safe and smooth transition for kidney and liver transplant recipients from hospital discharge to home. This role involves direct patient education, medication review, and instruction, along with coordination across multidisciplinary teams to optimize education for post-transplant success.

KEY RESPONSIBILITIES:
MINIMUM EDUCATION REQUIRED:
Graduate from an accredited school of nursing
MINIMUM EXPERIENCE REQUIRED:
Two years of nursing experience required.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Registered Nurse licensure in the State of Georgia.
ADDITIONAL QUALIFICATIONS:
Two years of critical care or procurement experience preferred. CCTC, CCPC, or CCTN preferred.
