Central Coast Home Health & Hospice is a fast-growing, locally owned and operated organization with an excellent reputation throughout the Central Coast. We are now seeking a full-time or part time   Medical Social Worker or Social Worker Assistant to work with both Home Health and Adult Hospice patients in the Northern San Luis Obispo area (Atascadero, Templeton, Paso Robles). 

This role offers the opportunity to make a meaningful impact while enjoying autonomy and flexibility, supported by clinical supervision from an experience LCSW. Clinical supervision is available for candidate working toward licensure.

Requirements:

• Bachelor's Degree in Sociology, Psychology, Human Development OR
• Master's Degree from a school accredited by the Council on Social Work Education  

• Minimum of one-year social work experience in a healthcare setting, home health &/or hospice experience preferred.
• Understand the "hospice philosophy" and issues of death and dying
• Reliable vehicle to drive for work
• Current Driver's license
• Clean background & driving record
• Maintain auto insurance coverage
• CPR certified
