Shift(s) Description : First shift, Monday–Friday, 8:00 AM to 5:00 PM
Weekend Requirements : No weekends required
On Call Requirements : No call required

• Provide outpatient palliative care services including home visits, extended care facility consultations, telehealth, and clinic-based care.
• Manage 6–8 patients per day based on location across a 9-county area in Ohio.
• Participate in delicate, goals-of-care discussions with patients and families facing serious or terminal illnesses.
• Assess, plan, implement, and evaluate individualized care needs in alignment with palliative care principles.
• Collaborate with a supportive interdisciplinary team including other NPs, physicians, and administrative staff.
• Participate in care transitions, including determining when patients would benefit from continued palliative care or hospice services.
• Work closely with a hospitalist team responsible for all patient admissions.
• Document patient encounters thoroughly and maintain excellent communication with patients, families, and care teams.
• Not accepting Locums candidates.
• Not accepting new graduate candidates - experience required.

REQUIREMENTS:

• Board Certified NP - REQUIRED.
• OH license (or willing to obtain) - REQUIRED.
• Experience as advanced provider in an acute care setting or with palliative care and hospice (NO EXCEPTIONS) - REQUIRED.
• Clean malpractice and license - REQUIRED.
