Overview
Join our client's dedicated care team as a Home Health Physical Therapist. This part-time weekend role offers the opportunity to provide exceptional patient care in a collaborative and supportive environment. The position supports patients in Jackson, MI, and surrounding areas, offering flexible scheduling and strong professional development resources.
Key Responsibilities
Provide management and delivery of patient care plans
Conduct comprehensive patient assessments and implement individualized therapy plans
Collaborate with interdisciplinary team members to ensure continuity of care
Maintain timely and accurate documentation in EMR systems
Educate patients and families on therapy techniques and home exercise programs
Promote patient independence and optimal functional outcomes
Schedule and Shift Details
Part-time, Saturdays and Sundays, every other weekend
Travel
Local travel to residential homes
Qualifications
Required
Bachelor's degree or higher in Physical Therapy from an accredited institution
Valid Physical Therapist license in Michigan (MI)
Minimum 1 year of healthcare experience
Experience with Electronic Medical Record (EMR) systems
Valid driver's license and reliable transportation
Preferred
