Salinas Valley Post Acute is seeking a Social Services Assistant to join the team full time.
Starting Rate: $23 per hour
As a Social Services Assistant, you will play a vital role in supporting the emotional, social, and physical well-being of residents under the direction of the Social Service Director. This position requires a compassionate, organized, and resourceful individual to help manage and deliver social services programs and resources.
Key responsibilities:
- Resident and family support:
- Assess residents' social and emotional needs and assist them in adjusting to their institutional life.
- Help residents achieve the highest possible level of independence and self-care.
- Provide emotional support to residents and their families coping with illness, stress, and anxiety.
- Administrative and care coordination:
- Maintain detailed and regular social service progress notes.
- Assist with discharge planning, including coordinating with outside agencies and arranging for necessary equipment and services.
- Schedule on-site and off-site patient appointments for services like optometry, podiatry, and dentistry.
- Perform administrative duties, such as completing forms and reports.
- Resource and program management:
- Inform residents and families about available financial programs, including Medicare and Medicaid.
- Connect residents with community health and social welfare resources.
- Assist in developing and implementing social service policies and procedures.
- Participate in community planning and department meetings.
Qualifications
- Education: A high school diploma or GED is required. An Associate or Bachelor's degree is preferred.
- Experience: Prior experience in a healthcare setting is preferred.

- Strong written and verbal communication abilities.
- Ability to interpret documents and regulations.
- Excellent problem-solving and reasoning skills.
- Proficiency with standard office equipment and computer use.
