Liberty Healthcare is now hiring for a full-time Incident Review Specialist position.
As an Incident Review Specialist, you will be an active contributor with a large-scale program that seeks to protect and ensure the quality of services and supports provided to older adults and people who are living with developmental disabilities.
You will be an integral part of Liberty Healthcare’s agile team which focuses on initiatives related to critical incident investigations and reporting; continuous quality management; data systems; and HCBS provider oversight, technical assistance, and training.
Your typical workday in this role will involve traveling to residences and service locations throughout the region and conducting investigations of reported adverse incidents related to the home and community based care of people who are living with disabilities.
Your specific responsibilities will include:
The downstream impact of your work will contribute to the advancement of systemic quality improvement initiatives that positively impact the health and well-being of a significant number of older adults and people who have intellectual and developmental disabilities.
This is a hybrid position which will allow you to work partially from your home office, and also require you to travel regularly throughout the region. You will enjoy a high degree of autonomy and work a variable schedule which may include occasional evening hours and weekends.

You are a good fit for this position if you possess the following:
You have relevant work experience and transferrable skills if you’ve worked as a Social Worker, Investigator, Service Coordinator, Case Manager, Case Worker, Program Specialist, Human Services Specialist, Care Navigator, Client Advocate, Detective, law enforcement professional, or Quality Specialist / Quality Improvement professional.
Since this position requires regional travel, you must have your own vehicle and current auto insurance.
